I've been doing some maintenance on my OH's bike and noticed an interesting fault. It's a flat bar Road bike with a 9 speed tiagra setup. Running up and down the rear block, it consistently skips the second largest sprocket, going either way. I could understand it one way- I'd just need to adjust the cable accordingly. But both ways?

If careful adjustment doesn't work, strip cable and lubricate.
If that doesn't work, check vertical alignment of rear mech and consider a gentle bend...

    In indexed mode?
Try the B-tension adjustment screw, in case it's not persuading the chain in the right way. Perhap's it's a freak occurrence whereby that sprocket is misaligned relative to the other ones so that it has pokey teeth poking into the bushed bits of the chain rather than into the gap bits.
