It soon became useful and entertaining. There are regular posters, people who add useful info occasionally and plenty more who drop by to watch. That's fine. If you want to add news/comments it's easy to register and become a member.

Note I removed any data which was in the test form as some of the questions were slightly reworded. Suggest we use the start date of today (Monday 5th March).

I've submitted one that took place today. It's at the trivial end of the spectrum, but was still pretty intimidating.
I'm late to this party, and haven't followed the discussion to date, so this may already have been covered, but I didn't spot the option to say when this happened (i.e. a date, as opposed to a time). That matters a bit, I reckon, because clearly 'no lights on' in evening rush hour in December is a different story to evening rush hour in June.

Greenroofer - the form automatically stamps submissions with the time and date, so I've assumed that people will submit forms vaguely near the date of the incident and we can get at the information that way. I'll admit that at least half the reason is that google forms is rubbish at dates!
Do you still have the test records somewhere so that I can re-enter mine?
Wingpig, might be easier if we take a start date of yesterday for incidents logged. With my statistical hat on, I'd be concerned that if not everyone will naturally add in their worst incident from the last year and the list will make it look like armageddon out there. Thoughts?

Ah well done for finding it. It was supposed to be sticky but refused to stick, I guess it hasn't been used much since it is difficult to find.
Part of the problem was I never did a permanent link as I had intended.
