I'm sure someone posted recently a URL for an incident report template? As I recall it was from another forum but contained prompts for all the relevant bits to make up a complete report - the idea being that it aided the police, thus making following up on it as easy as possible.

Needless to say, I now can't find it, after being endangered by a driver who was busy texting as well as driving...

That might have been Stop Smidsey. These are passed on to Police forces who are interested. Unfortunately, as far as I can recall, only London Police are actually interested..

However the point of the website it to gather data to try and make them be interested.

I'd checked there, but I was looking for a fill-in-the-blanks type report. I was (travelling somewhere) when (something happened). The vehicle was a (purple one with spots) driven by a (circus clown). The weather was (raining with a fetching pink haze) and visibility was (flashing cateyes all round).

That sort of thing - I know I came across one recently, but now can't find it at all.

    Probably doesn't matter...

Exactly the one! I knew I wasn't dreaming (or at least I hoped I wasn't), but couldn't find it via lots of different advanced searches, and searching the web for the original was fruitless since I couldn't remember the host forum...

Hopefully I won't be too late tomorrow morning to buy you a coffee!
